Troubleshooting - Initial Research

In order for the user to research an issue, first they need to understand how the company is setup in bC4 and ACAManager to determine if the issue is occurring due to the setup or the data itself.
The following areas should be reviewed prior to looking at the employee level:

bC4:

  • Is the plan self-funded?
  • YES – Part III (1095C) or Part IV (1095B) will populate if an EE is enrolled
  • NO – Part III (1095C) or Part IV (1095B) will not populate
    • Is the plan age-rated?
  • YES – Confirm that there are no missing age-banded rates
  • NO – Move on to next question
    • Is the lowest cost plan less than 9.5% of monthly FPL ($93.18)?
  • YES – Expect to see 1A if EE is eligible
  • NO – Expect to see 1E if EE is eligible
    • Does the plan have a waiting period?
  • YES – Expect 2D for the month(s) new hire employees are in their waiting period
  • NO – Expect 2H/2G for months not enrolled and 2C for months enrolled
    • Is the eligibility of the plan based on UDF?
  • YES – What is the criteria for EE to be eligible for the plan?
  • NO – Move on to next question
    • Is the eligibility of the plan based on number of hours worked per week?
  • YES – What is the number of hours?
  • NO – Move on to next question
    • Is the eligibility of the plan based on EE status?
  • YES – What is/are the status(es) required for eligibility?
  • NO – All statuses are eligible

ACAManager as Admin:

  • Is the group “Reporting Only”?
  • YES – The eligibility will be based on the FT status in bC4
  • NO – Expect to see the monthly hours uploaded in ACAManager
    • Is the group setup as Hybrid Control Group?
  • YES – Confirm that the divisions in bC4 have their own Tax ID Numbers and that they are not the same as the parent company
  • NO – Move on to next question
    • Is the group setup as Control Group?
  • YES – Is the Control Group added and linked to the group in Control Group field?
  • NO – Move on to next question
    • Is the group setup as Multi-Employer?
  • YES – Confirm that the divisions in bC4 have their own Tax ID Numbers and that they are not the same as the parent company
  • NO – Move on to next question

ACAManager as Broker:

Review Lookback Measurement under Company Administration/Company Setup

  • Is the group using FPL?
  • YES – Expect to see 2G
  • NO – Expect to see 2H
    • Is the group using Union flag?
  • YES – Expect to see 1H/2E for employees marked “Yes” as Union Members
  • NO – Expect employees marked “Yes” as Union Members to reflect the same codes as any non-union employees

Review Company Plans

  • Is/Are the plan(s) MEC?
  • YES – Expect to see MEC checked in EE Profile page if eligible for plan
  • NO – Expect to see MEC not checked in EE Profile page if eligible for plan
    • Is/Are the plan(s) MVS?
  • YES – Expect to see MVS checked in EE Profile page if eligible for plan
  • NO – Expect to see MVS not checked in EE Profile page if eligible for plan

Review Multi-Employer Group Setup if applicable
Each division have its own Company Classification

  • Is the division classified as ALE?
  • YES – 1095C form
  • NO – 1095B form
